New Horizons Clinic-Based Services
New Horizons Clinic-Based Services provides an array of outpatient therapeutic services for youth and their families with mental health, substance use and/or co-occurring disorders.
Services may include:
- Clinical assessments and diagnoses to determine the needs, strengths and preferences of clients
- Development of treatment goals and plans in collaboration with clients
- Court-ordered mental health and substance use evaluations and treatment reports
- Individual, family and group therapy at CS office sites
- Education and skills development to build successful problem solving, teamwork and positive communication
- Collaboration and consultation with probation officers, school staff, social workers, physicians, psychiatrists, other professionals and others providing or potentially providing resources
- Crisis intervention
- Medication evaluation and management in conjunction with treatment services
- Discharge planning for clients in psychiatric hospitalizations and residential treatment centers
- Linking youth and families to community resources, including self-help groups (i.e., A.A., N.A., Alateen, Families Anonymous)
