New Horizons Case Management Services
New Horizons Case Management Services provides case management for youth and their families with serious mental health, substance use and/or co-occurring disorders needing service coordination, crisis intervention, and discharge planning services.
Services may include:
- Active case management, including FAPT case management and medication management
- Clinical assessments and diagnoses to determine the needs, strengths and preferences of clients
- Develops treatment goals and plans in collaboration with clients
- Specialized psychoeducation services (i.e., behavior management, parenting skills, positive communication, anger management, substance use prevention and education, problem solving skills) to build positive communication and behavior skills
- Consults and collaborates with probation officers, social workers, psychiatrists, school staff, professionals and other community stakeholders
- Connects families to community resources to address the youth’s problems, needs and interests
- Crisis intervention
- Develops a team with family, friends, professionals and other supports to promote self-sufficiency and resiliency
- Discharge planning for clients in psychiatric hospitalizations and residential treatment centers
- Referrals to mental health, substance use, and co-occurring disorders residential treatment programs
