New Horizons Home-Based Services
New Horizons Home-Based Services provides intensive, in-home therapeutic services for youth at risk of out-of-home placements due to serious mental health, substance use and co-occurring disorders as well as crisis intervention, discharge planning, and case management services.
Services may include:
- Intensive individual and family therapy, generally conducted several times per week in the client home, school, office, and other community locations
- Clinical assessments and diagnoses to determine the needs, strengths and preferences of clients
- Development of treatment goals and plans in collaboration with clients
- Specialized treatment and skill building (i.e., behavior management, parenting skills, positive communication, anger management, substance use prevention and education, problem solving) to help youth and families build positive communication and behavior skills
- Connecting families to community resources to address the youth’s problems, needs and interests
- Collaboration and consultation with other professionals working with the family
- Crisis intervention
- Case management to include FAPT case management
- Developing a team with family, friends, professionals and other supports to promote self-sufficiency and resiliency
- Discharge and after-care planning, including as step-down from residential treatment center placements and step-down from intensive in-home services to outpatient therapy
- Discharge planning for clients in psychiatric hospitalizations and residential treatment centers
