Supported Living Services
The Supported Living Services program assists adults with serious mental illness and/or co-occurring disorders, who live in the community and require community- based services to achieve and maintain their maximum level of independence. The program is intended to prevent psychiatric hospitalizations, decrease involvement with the criminal justice system, promote successful community living and foster maximum independence. The primary services provided include Community Case Management and Mental Health Skill Building.
Assertive Community Treatment (ACT)
To assist adults with serious mental illness who are best served in the community to reach and maintain their maximum level of independence. This is accomplished through an array of mental health services provided outside the clinic/office setting to permit or enhance the individuals functioning in the community.
Care, Treatment, Training, or other supports provided:
- Diagnostic and ongoing assessment to ascertain the needs, strengths and preferences of the individual
- Development of treatment goals and plans
- Case management/Care Coordination
- Nursing
- Support for wellness self-management, including the development and implementation of individual recovery plans, symptom assessment, and recovery education
- Psychopharmacological treatment, administration and monitoring
- Substance abuse assessment and treatment for individuals with a co-occurring diagnosis of mental illness and substance use disorders
- Individual supportive therapy
- Skills training in activities of daily living, social skills, interpersonal relationships and leisure time
- Supportive in-home services
- Work-related services to help find and maintain employment
- Support for resuming education
- Support, education, consultation, and skill-teaching to family members and significant others
- Collaboration with families and assistance to individuals with children
- Direct support to help individuals secure and maintain decent, affordable housing that is integrated into the broader community and to obtain legal and advocacy services, financial support, money-management services, medical and dental services, transportation, and natural supports in the community
- Mobile crisis assessment, interventions to prevent or resolve potential crises, and admission and discharge planning for inpatient psychiatric treatment
