New Horizons Probation Services provides a range of therapeutic services to youth on probation and their families with mental health, substance use and co-occurring disorders. Services are provided at CS offices, probation offices, on-site at the Juvenile Detention Center, client’s home and in the community.
Services may include:
- Individual, family and group therapy
- Clinical assessments and diagnoses to determine the needs, strengths and preferences of clients
- Development of treatment goals and plans in collaboration with clients
- Crisis intervention
- Court-ordered mental health and substance use evaluations and treatment reports
- Intensive care coordination to assess needs, identify and link to resources, and coordinate services
- Education and skills development to build problem solving skills, teamwork and positive communication skills
- Collaboration and consultation with probation officers, school staff, social workers, physicians, psychiatrists, other professionals and others providing or potentially providing resources
- Case management, including FAPT case management, active case management and psychotropic medication case management
- Discharge planning for clients in psychiatric hospitalizations and residential treatment centers
- Linking youth and families to community resources, including self-help groups (i.e., A.A., N.A., Alateen, Families Anonymous)
